Abstract: 

Introduction: Vaginal birth after cesarean is one of the methods used to reduce the increasing number of cesarean sections. This study was performed with aim to evaluate maternal and FETAL COMPLICATIONS in women who had successful vaginal delivery after cesarean. Methods: This retrospective study was performed on 160 women who had successful vaginal delivery after one cesarean section with transverse uterine incision at Mashhad Omolbanin hospital in 2016-2018. Demographic data related to delivery and the infants' data were collected from the patients' records. Data was analyzed by SPSS software (version 11) Kolmogrov-Smirnov, Chi-square and t-test. p < 0. 05 was considered statistically significant. Results: Among 160 patients, 141 cases (88. 12%) had successful normal vaginal delivery and 19 cases (11. 88%) had cesarean section. The two groups of success and failure of normal delivery after cesarean section had no significant difference in terms of mean maternal age (p= 0. 18), body mass (p=0. 07), gestational age (p= 0. 15) and distance from previous cesarean section (p=0. 07). The bishop score less than 6 was significantly higher in the failure group than in the success group (p=0. 02). In the failure group, postpartum hemorrhage (p= 0. 03) and NICU admission (p=<0. 001) were significantly higher. Conclusion: Vaginal delivery after cesarean is a safe delivery method and reduces maternal and FETAL risks caused by repeated cesarean section.

Abstract: 

Introduction: The maternal perception of FETAL movements at term has been used as an evaluative test of FETAL wellbeing since the last 30 years, but there are different reports about its value. The aim of this study was evaluation of decreased FETAL movement and relation to situation of fetus in uterus. It is significant that we understand the relationship between decreased FETAL movement and increased FETAL distress, heart deceleration and meconium. Methods: This study was carried out from 2000-2001 in Kerman Bahonar hospital. The method was a case control one with 100 cases and 100 controls. Cases and controls were selected from pregnant women entering active phase of labor or induction of labor for causes other than perception of decreased FETAL movements. FETAL heart rate was monitored in both groups during labor phase and also, color of amniotic fluid and meconium during rupture of membrane was recorded. Type of delivery, Apgar scores at minute 1 and 5, mortality and morbidity were compared in both groups. In this study, amniotic fluid volume was a significant variable that was paid special attention. Results: After analysis , there was a significant relationship between decreased FETAL movement and many variables, for example, gravidity , deceleration of FETAL heart , 1st & 5th minutes apgar score, FETAL mortality, type of delivery , amniotic fluid volume and meconium staining . Conclusion: We can conclude that decreased FETAL movement is a significant factor in evaluation of FETAL well being in near-term ante partum cases.

Abstract: 

Introduction: Gestational diabetes is a common complication during pregnancy, affecting maternal and FETAL outcomes. Considering the increasing prevalence of gestational diabetes around the world, in the present study, we aimed to assess FETAL and maternal outcomes in pregnant diabetic women.Methodology: In this cross-sectional study, 85 women with gestational diabetes (gestational age>28 weeks), admitted to the maternity ward, were enrolled, along with 160 women with a normal pregnancy, who met the inclusion criteria (i.e., no history of diabetes, chronic diseases, endocrine disorders, or steroid use). By using a questionnaire, the required information, i.e., demographic data, medical information, gestational age, Apgar score, and adverse pregnancy outcomes such as preeclampsia, premature rupture of membranes, postpartum hemorrhage, macrosomia, stillbirth, and hypoglycemia, was recorded. The gathered data were analyzed using SPSS.Results: The mean age of diabetic and healthy mothers was reported to be 30.4±5.01 and 27.2±4.03 years, respectively; a statistically significant difference was found between the two groups (P<0.02). Also, the meansystolic and diastolic pressure, body mass index, and cesarean section were more prevalent in diabetic mothers (P=0.01); the two groups were significantly different in this regard. Furthermore, FETAL COMPLICATIONS such as macrosomia, stillbirth, and hypoglycemia were more common among diabetic subjects, compared to their healthy counterparts (P<0.01).Conclusion: Among maternal and FETAL COMPLICATIONS, gestational hypertension and cesarean section were more common in diabetic cases. Also, FETAL COMPLICATIONS such asmacrosomia, stillbirth, and hypoglycemia were more prevalent in newborns of diabetic women. Therefore, implementation of educational programs, screening of diabetic mothers, and attention to the associated COMPLICATIONS seem essential. Overall, all these COMPLICATIONS highlight the importance of gestational diabetes prevention.

Abstract: 

Preeclampsia is a major unsolved obstetrical problem and a contributor to maternal and perinatal mortality and morbidity. The aim of this study was to determine the prevalence of preeclampsia and its correlation to maternal and FETAL COMPLICATIONS in women referred to Emam Sajjad hospital of Yasuj for delivery or termination of pregnancy. The study was carried out on 1000 women who delivered from summer to autumn of 2001 in that hospital. Data were collected by performing interviews and physical examinations from women in order to recognize signs and symptoms of preeclampsia with regard to relevant labratory test results.Prevalance of preeclampsia was found to be 5.4%.The rate of maternal and FETAL COMPLICATIONS in preeclamptic women were significantly higher in comparison with others who delivered in the same period and included: abruptio placentae (14.8%) Cesarian section (29.6%), eclampsia (9.2%), preterm labor (30.2%), lowbirth weight (23.5%), stillbirth (11.1%), FETAL distress (27.8%) and low Apgar score (22.2%), (P<0.0001). Maternal age over 35 years old, maternal diabetes and history of preeclampsia in previous pregnancies were risk factors for preeclampsia in this study. Considering the above findings, the prevalence of preeclampsia in our study was in the same range as that reported from other parts of the world .Since the maternal and FETAL COMPLICATIONS of preeclampsia in the study area is significant, therefore mothers with preeclampsia have to be identified early by promoting prenatal care in local heath centers.
